General Insurance Requirements for All Rentals
Insured and Customer Rental Account Name Must Match Exactly.

1. Commercial general liability insurance necessary to protect Kirby-Smith Machinery, Inc. and its affiliates during the term of the rental from any and all claims for bodily injury, death or property damage arising from the operation, handling and transportation of the rented equipment with limits of not less than one million dollars ($1,000,000.00) combined single limit per occurrence and general aggregate per location or job site. Kirby-Smith Machinery, Inc. must be endorsed without limitation, condition or exclusion as additional insured.
2. All risk physical damage insurance for the replacement value of the equipment with a deductible acceptable to Kirby-Smith Machinery, Inc.  Kirby-Smith Machinery, Inc. must be listed as the Loss Payee.
3. Workers compensation insurance written to comply with all laws in the states in which the equipment shall be used or operated.

Additional Requirements for Crane Rentals

Certificate must state NO BOOM or OVERLOAD DAMAGE EXCLUSIONS to the physical damage insurance. This coverage may be written on a blanket or a machine specific basis. In the latter case the following information for the rented equipment must be listed on the certificate:

• Make
• Model
• Year
• Crane Serial Number        
• VIN Number 
• Machine Value
